Description (Service)
  • Assists stroke clients with connecting to required services and programs in the community 
  • Provides accompaniment to appointments and other activities/services 
  • Offers referrals to Timiskaming Home Support services and/or other community agencies
 
Assistance is provided in accessing the following services which can include but are not limited to: 
  • Applying for an appropriate residence such as supportive or permanent housing 
  • Finding home renovation/modification services 
  • In-home nursing, personal care and housekeeping 
  • Physical/recreational activities/programs
Hours Office Mon-Fri 8:30 am-4:30 pm
Areas Served Englehart ; Kearns ; Kirkland Lake ; Larder Lake ; Matachewan ; Virginiatown
Eligibility Anyone who has suffered a stroke, in need of assistance in recovery process
Application
  • Contact the Stroke Community Navigator for initial consultation to discuss which programs/services would be beneficial 
  • Meeting will be arranged with client and family to discuss options and to complete application form 
  • Upon acceptance for services, assistance provided to complete a plan for goals and recovery
