Description (Service)
  • Provides information, advocacy and support to assist individuals to remain in their homes as long as possible 
  • Acts as respite for informal caregivers such as extended family 
  • Coordinates group activities such as bingos, indoor/outdoor games, and presentations on various topics of interest
 
Support Services offered include: 
  • Caregiver support 
  • Case management 
  • Congregate dining 
  • Friendly visiting 
  • Service arrangement and coordination 
  • Weekly visiting for social and safety reasons
Hours Mon-Fri 9 am-4 pm

Eligibility
  • Focus is on frail, vulnerable and at-risk elderly (55 years and older) adults and individuals trying to overcome barriers in accessing care such as low income, geographic isolation, language or disability issues or 
  • Off-reserve Status, Non-Status, Inuit and M├ętis individuals who require assistance to remain in their home as long as possible
