Description (Service)
  • Provides a range of community-based treatment, rehabilitation and support services 
  • Focuses services around personal recovery goals to promote capabilities and successful community living
Services are provided to identified clients by a combination of the following: 
  • general practitioner 
  • mental health worker 
  • nurse 
  • occupational therapist 
  • peer support worker 
  • psychiatrist 
  • social worker
Services offered to identified clients can include: 
  • 24 hour crisis response 
  • assessment, skill teaching, direct assistance in daily living, vocational, social, interpersonal relationships and leisure activities 
  • consultation, support and education to family and friends 
  • individual supportive therapy 
  • medication administration and monitoring  
  • peer support 
  • prescribes medication  
  • service coordination and direct assistance in obtaining the basic necessities of life 
  • substance abuse services 
  • symptom assessment, management

Eligibility Ages: 16 year(s) and up 
  • Individuals who frequent hospitals for treatment related to a serious mental illness (e.g. main diagnosis of schizophrenia or bipolar disorder) 
  • Examples of eligible individuals can include those who do not take medication as required or who have not benefited from traditional services
  • Intake assessment required  
  • Approval for services is determined through collaboration between treatment staff, referring source and applicant
