Description & Services
Description (Service) Self-Management and Exercise Program for stroke survivors and their care partners (family or friends)
 
  • Focuses on education, goal-setting, and exercise 
  • Helps individuals learn about strategies to manage the daily challenges of living with a stroke 
  • Holds sessions three times a year, in Jan, Apr, and Sep  
  • Provides up to 6 participants in Thunder Bay and 2-6 participants at regional sites via Ontario Telemedicine Network
Hours Tue, Fri 10:30 am-12:30 pm

Eligibility
  • Stroke Survivors and their family or friends 
  • Must have completed all rehabilitation
Application Client or health care provider referral through Outpatient Neurology Physiotherapy
 
Note: The program coordinator will get the physician referral and clearance for exercise
