Diabetes Clinics offered 2nd Tue of every month; call Health Centre for times and to book appointment
Description (Service) Provides education and services for the prevention, management and treatment of diabetes  
  • Coordinates monthly clinics provided by Diabetes Health Thunder Bay at the FWFN Health Centre 
  • Provides case management and screening services  
  • Works to increase early detection and to reduce the number of avoidable complications that can arise from uncontrolled diabetes  
  • Advises on developing and implementing an approach to care and management of diabetes  
  • Promotes increased physical activity and knowledge of healthy eating  
  • Provides workshops, presentations and hands-on training for cultural activities and traditions  
  • Assists individuals in finding appropriate services  
  • Provides referrals to other health care professionals and the Northern Diabetes Health Network (NDHN)
Hours Health Centre Mon-Fri 9 am-12 noon, 1 pm-4 pm 
Diabetes Clinic First Tue of month 9 am-4 pm

Eligibility Members of Fort William First Nation living with diabetes or at-risk
Application Diabetes clinics require booked appointments
