Description (Service) Offers a variety of in home professional care services, including: 
  • Nursing (acute and chronic care) 
  • Physiotherapy 
  • Occupational Therapy 
  • Social Work 
  • Speech and language pathology 
  • Nutritional and dietary 
  • School Health Support
In home needs assessments are performed by care managers, with appropriate services provided in partnership with other health care professionals and organizations in the community and region
