Description (Service)
  • Assists persons and their families who struggle with persistent and severe mental illness to achieve their goals, to feel as well as possible, and live in the community in a place of their choice 
  • Conducts person-centered planning to determine goals and supports
Offers the following programs and services:
Intensive Case Management Program 
  • Provides flexible and individualized support  
  • Develops supportive relationships 
  • Prevents the need for future hospitalization 
  • Offers 24-hour availability 
  • Provides community-based support
Dual Diagnosis Program 
  • Provides access to specialists via videoconferencing and in person consultations with visiting psychiatrists 
  • Offers support for families and front line staff who work with this population 
  • Develops strategies to effectively support individuals in a community setting 
  • Advocates for individuals and families to receive needed services 
  • Supports access to resources to improve quality of life 
  • Provides psychological education for individuals, families, caregivers and service providers

  • Housing Subsidy Program 
  • Community Wellness Project
Hours Office: Mon-Fri 8 am-4 pm 
Support: Mon-Sun 7 am-11 pm
