Description (Service)
  • Offers 6-week workshops for individuals with chronic or long-term health conditions such as diabetes, arthritis, stroke, cancer, colitis, lung disease, fibromyalgia, Parkinson's, and heart disease 
  • Workshops are offered in-person as well as by videoconferencing
 
Workshop topics include: 
  • Pain and fatigue management 
  • Developing action plans and problem solving skills 
  • Exercise and nutrition 
  • Managing Medications 
  • Making treatment decisions

Areas Served Kenora District ; Rainy River District ; Thunder Bay District
Eligibility Ages: 18 year(s) and up 
 
  • Individuals with chronic or long-term health conditions and their caregivers 
  • Participants must be able to set goals and function in a group setting. For example, the program would not be appropriate for someone with significant dementia or memory impairment, or for someone who would be disruptive in a group
