Description (Service) Nurse Practitioner-led medical clinic 
  • Nurse practitioners (NP) are Registered Nurses with advanced university education who provide personalized, quality health care to patients 
  • Works in partnership with Nurses, Registered Practical Nurses, Dietitians, Social Workers and collaboratively with Physicians
 
Offers a variety of health care services including: 
  • focus on chronic disease management 
  • counselling and education 
  • diagnose and treat illness and/or injuries 
  • order and interpret diagnostic tests 
  • perform physical check-ups 
  • referrals to family physicians, specialists and other health professionals 
  • supportive care through illness 
  • treatments and/or procedures  
  • write prescriptions
Hours Mon-Fri 9 am-5 pm

Eligibility Ages: 16 year(s) and up 
 
  • Have no primary care provider 
  • Have a chronic disease which requires on-going medications and monitoring by a health care provider
