Description (Service)
  • Provides government funding for eligible individuals to access physiotherapy services  
  • Services are provided by various community clinics

Hours Service Ontario INFOLine Mon-Fri 8:30 am-5 pm
Areas Served Algoma District ; Cochrane District ; Kenora District ; Manitoulin District ; Nipissing District ; Rainy River District ; Sudbury District ; Thunder Bay District ; Timiskaming District
Eligibility Individuals who: 
  • Are age 19 or younger 
  • Have been discharged from a hospital after an overnight stay for a condition requiring physiotherapy 
  • Receive income from Ontario Works (OW) or Ontario Disability Support Program (ODSP) 
  • Are age 65 or older 
  • Are age 65 or older and require in-home physiotherapy (whether in own home or retirement home) 
  • Are age 65 or older and living in a long-term care home
 
Note: If the individual is not listed above then the government will not cover their physiotherapy costs
Application  
The individual must contact their doctor or nurse practitioner for a referral if they: 
  • Are age 19 or younger 
  • Have been discharged from a hospital after an overnight stay for a condition requiring physiotherapy 
  • Receive income from Ontario Works (OW) or Ontario Disability Support Program (ODSP) 
  • Are age 65 or older
 
If the individual is aged 65 or older and requires in-home physiotherapy (whether in own home or retirement home), contact the Local Health Integration Network (LHIN) in your community
 
If the individual is aged 65 or older and living in a long-term care home, contact the doctor or nurse practitioner on staff
