  • Provides care and support to individuals experiencing a serious mental illness 
  • Focuses on maintaining or increasing an individual's ability to care for themselves, wellness planning, skill building, placement, and follow-up 
  • Supports individuals as they transition back into the community 
  • Offers skill development and education to individuals, families, and the community

Eligibility Ages: 18 year(s) and up 
 
Individuals must: 
  • Have a diagnosis of a serious mental illness 
  • Be medically stable 
  • Have exhausted district level services 
  • Have a signed letter of commitment from the referring agency/service
