Description (Service) Delivers health and related programs and services to member First Nation communities
 
Community Health Nursing Program 
  • Delivers health care through clinics, health centres and nursing stations  
  • Focuses on health promotion, prevention, immunization, communicable disease control, health teaching, home visits, community visits, prenatal and post natal teaching, well baby clinics, school health, adult health, chronic disease and long term care
 
Community programs and services include: 
  • Clinical care services, administered through the N'Mninoeyaa Community 
    Health Access Centre 
  • Community Support Services 
  • Diabetes Program 
  • Fetal Alcohol Spectrum Disorder programs 
  • Mental Health Program 
  • Traditional Health Program

Areas Served Atikameksheng Anishnawbek First Nation ; Batchewana First Nation ; Garden River First Nation ; Mississauga First Nation ; Sagamok Anishnawbek First Nation ; Serpent River First Nation ; Thessalon First Nation
Boundaries First Nations between Sudbury and Sault Ste. Marie - along the Highway 17 Corridor
Eligibility Must belong to one of the member First Nations and includes the urban First Nations population of Sault Ste Marie
Application Services are delivered and accessed on-site at each of the 8 partner health centres and at community level through Health Centres
Languages English ; Interpretive Services - Ojibway, available upon request
