Description (Service)
  • A 10-bed supportive housing program with 7 transitional and 3 long term beds for adults with a psychiatric diagnosis 
  • Assists the residents in working towards their optimal level of functioning as a transition to more independent living 
  • Provides individual life skills programming in areas such as money management, nutrition, meal preparation, personal hygiene, medication routine, leisure/recreation counselling, safety and social skills 
  • Links residents to a variety of community resources  
  • Facilitates discharge planning

Eligibility Ages: 16 year(s) and up 
 
  • Must have a psychiatric diagnosis and be under the care of a physician and treatment team 
  • Must have the capacity to adapt to living cooperatively with others 
  • Cannot be a threat to the general safety of others 
  • Must have control of substance abuse if applicable
Application
  • Referrals by any health professionals, hospitals, social community service agencies or family members 
  • Referrals reviewed by a selection committee
