Description (Service) Assists individuals with a primary diagnosis of severe and persistent mental illness to live independently in the community 

 
Case Management 
  • Works with consumers to improve their quality of life and level of wellness 
  • Provides services in a place of consumer choice, predominantly in the community 
  • Focuses on individuals with mental illness who are homeless or at risk of becoming homeless
 
Provides assistance in the following areas: 
  • Interpersonal support 
  • Improving living skills  
  • Consultation and assessment 
  • Coordination and advocacy 
  • Financial assistance and budgeting 
  • Family support 
  • Crisis support 
  • Supportive counselling  
  • Medication monitoring
 
Community Treatment Orders (CTO) 
  • Provides a treatment and service plan issued by a physician in consultation and coordination with the consumer and service providers 
  • Provides case management services and support
 
Supported Housing 
  • Empowers consumers to live in accommodations of their choice 
  • Offers flexible and individualized supports that vary in intensity 
  • Assists individuals in acquiring and maintaining housing
Hours Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm
