Description (Service) Supports for individuals with prediabetes or diabetes
Offers a diabetes management clinic to assist individuals in understanding and managing their illness
Programs and services include: 
  • Client support groups 
  • Community screening 
  • Exercise programs 
  • Foot care workshops/clinics 
  • Health fairs 
  • Presentations to the public and to health care providers 
  • School and daycare programs and visits
Walk-In Clinic 
  • Provides consultations and services for a half day per week on a walk-in basis
Program services are provided through: 
  • Individual sessions 
  • Group sessions 
  • Health promotion activities
Hours Mon-Fri 8 am-12 noon, 1 pm-4 pm 
Walk-In Clinic Tue 8 am-11:30 am
Areas Served Balmertown ; Cochenour ; Ear Falls ; Madsen ; McKenzie Island ; Red Lake ; Starratt Olsen ; Wabauskang First Nation
Eligibility Adults 18 years of age and over 
  • Type 1 Diabetes 
  • Type 2 Diabetes (diet-controlled, pills, insulin or any combination of treatment) 
  • Gestational Diabetes 
  • Type 1 / Type 2 Pregnancy 
  • Impaired Glucose Tolerance 
  • Impaired Fasting Glucose 
  • High Risk
Child and Youth 17 years of age and under 
  • Type 1 Diabetes 
  • Type 2 Diabetes (on insulin) 
  • Type 2 Diabetes (no insulin)
  • Open to referrals from any source 
  • Booked appointments required (except Walk-In Clinic)
