  • Provides treatment for individuals whose needs are best met with services available from a broad range of clinical disciplines 
  • Promotes community integration, quality of life, meaningful outcomes in life and recovery 
  • Provides comprehensive individualized care which includes assessment, treatment and follow up care
Hours Mon-Wed 8 am-7 pm, Thu-Fri 8 am-5 pm

Eligibility Ages: 16 year(s) and up 
Have serious mental health problems, which may include one or more of the following: 
  • Mood disorders such as depression and bipolar affective disorder 
  • Anxiety disorders such as generalized anxiety disorder, panic disorder, agoraphobia, phobias, post traumatic stress disorder (PTSD), obsessive compulsive disorder 
  • Psychotic disorders such as schizophrenia, delusional disorder, schizo-affective disorder, and psychosis not otherwise specified 
  • Personality disorders 
  • Dual diagnosis (developmental delay and mental illness)
Have an identified need for one or more of the following: 
  • Psychiatric assessment, consultation or medication review 
  • Comprehensive assessment or treatment provided by various disciplines 
  • Are willing and able to engage in treatment and attend appointments 
  • Clients in active receipt of Workplace Safety and Insurance Board, Long Term Disability or private insurance will be considered for admission into the program on an individual basis
  • When emergency services are required and should be obtained at Thunder Bay Regional Health Science Centre (TBRHSC)  
  • When third party assessments are requested , such as independent medico-legal examinations 
  • When intensive case management is required  
  • When the referral is primarily for marital counseling  
  • When clients or their substitute decision makers are not willing to provide consent to treatment
  • Referrals are accepted from individual clients, physicians, nurse practitioners, family members and community agencies 
  • If the referral is not from a family doctor or nurse practitioner, the doctor or nurse practitioner should be aware of the referral since the client will be registered to the program under the family doctor or nurse practitioner
