Description & Services
Description (Service) Travels to small communities to provide diabetes education and support to individuals at risk of and who may be living with diabetes
Offers a Diabetes Mobile Health Team consisting of a Nurse Practitioner, Registered Nurse, Dietitian, Foot Care Nurse and Community Health Worker
Nurse Practitioner (NP) 
As the Primary Care Leader of the Mobile Team, the NP helps the patient manage this illness/disease by monitoring blood glucose levels, blood pressure and cholesterol, and will also: 
  • See the patient for physical checkups 
  • Order and interpret diagnostic tests 
  • Write prescriptions 
  • Provide supportive care 
  • Provide treatments and/or procedures  
  • Make referrals as necessary to the Norwest CHC Multidisciplinary Team, specialists and other health care professionals
Registered Nurse (RN) 
Works with individuals to: 
  • Monitor blood glucose, A1C levels, blood pressure, and weight  
  • Discuss ways to improve health outcomes
Works with individuals, groups and families to support healthy eating including: 
  • Nutrition counselling 
  • Weight management 
  • Meal planning 
  • Community activities
Foot Care Nurse 
  • Ensures individuals are maintaining good general foot health 
  • Monitors unexpected foot problems 
  • Makes referrals as necessary
Community Health Worker 
  • Offers health promotion resources and prevention programs to individuals or groups interested in achieving good health 
  • Works with individuals in finding and ensuring the appropriate and right supports are in place for health and social well-being
