Description (Service) Provides education and support for Diabetes, including the following: 
  • Community diabetes screening 
  • Counselling 
  • Elders outreach program 
  • Individual assessments 
  • Insulin starts and adjustments 
  • Learning to Live with Diabetes support group 
  • Nutrition sessions 
  • Presentations and workshops for Type 2 and Gestational Diabetes 
  • Prenatal and postnatal outreach and Elders outreach programs
Hours Mon-Fri 9 am-5 pm
Areas Served Thunder Bay
Eligibility Adult individuals with diabetes, pre-diabetes, or at-risk for the development of diabetes
