Description & Services
Description (Service) Supports for people living with pre-diabetes or diabetes
Offers a diabetes management clinic to assist individuals in understanding and managing their illness 
  • Provides access to consultations with registered dietitian  
  • Cooperates with assistance from registered nurse  
  • Offers basic, advanced and diabetic foot care 
  • Offers foot screening clinics  
  • Provides services of a chiropodist 3 days a month 
  • Provides referrals to other foot care service providers  
  • Coordinates group grocery shopping tours to help individuals make healthy food choices and to learn about reading labels  
  • Offers presentations and printed material for public education and awareness
Program services are provided through: 
  • Individual sessions 
  • Group sessions 
  • Health promotion activities
Hours Mon-Thu 8 am-3 pm, Fri 8 am-2:30 pm
Areas Served Anishinaabeg of Naongashing First Nation ; Couchiching First Nation ; Lac La Croix First Nation ; Mishkosiminiziibiing First Nation ; Naicatchewenin First Nation ; Nigigoonsiminikaaning First Nation ; Onigaming First Nation ; Rainy River First Nation ; Seine River First Nation ; Stanjikoming First Nation
Boundaries West to Big Island/Big Grassy First Nations, East to Lac La Croix First Nation, South to the US/Canada border, North to Onigaming First Nation

  • Gestational Diabetes
  • Type 1/Type 2 Pregnancy
  • Impaired Glucose Tolerance
  • Impaired Fasting Glucose
  • Pediatric Type 2
Application Accepts walk-ins or booked appointments; no referral required
Languages English ; Interpretive Services - Ojibway, available upon request
