Description (Service)
  • Provides case management support services to individuals who are in conflict with the criminal justice system 
  • Supports individuals who have been discharged from correctional facilities
 
Offers a variety of supports services including: 
  • Accessing employment or education, structuring daily activities 
  • Assistance with self care and daily living skills 
  • Education around diagnosis, symptom management 
  • Developing formal and informal support networks 
  • Housing access 
  • Assistance with community involvement/participation 
  • Support, problem solving, advocacy 
  • Aboriginal programming
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Thunder Bay
Eligibility Ages: 16 year(s) and up 
 
Individuals being discharged in various stages of the criminal justice system, including discharge from correctional facilities
Application
  • Self-referral, referral from family or a referring agency are accepted 
  • Referral form can be found on the website
