Description (Service)
  • Delivers sensitive, timely and portable services to individuals who are struggling with a serious mental illness or concurrent disorder 
  • Services are based on a psychosocial model of care which facilitate consumer's full and active participation in the recovery process and promotes independence 
  • Services are designed to maximize consumer's functioning in the community. 
  • Offers Case Management services which provide support services to people that emphasize choice, skills development, community access and consumer rights
 
Provides the following services: 
  • Assistance in accessing mental health and/or psychiatric services 
  • Help to integrate into the community advocating with informal and formal community resources 
  • Referrals to community agencies, where appropriate 
  • Assistance in accessing income support such as Ontario Works and Ontario Disability Support Program (ODSP) 
  • Housing rent supplement 
  • Assistance in obtaining and maintaining housing in the community
Hours Mon-Fri 8:30 am-4:30 pm

  • Individuals suffering from a mental illness and/or a concurrent disorder 
  • Resident of Thunder Bay 
  • Requires assistance and support in order to live successfully in the community
