  • Enrolment inquiries and registration for Ontario Health Cards that provide pre-medical and hospital care information regarding health services 
  • Processes Out-of-country claims 
  • Outreach services available to Northwestern Ontario residents 
  • Temporary residents are not covered

Eligibility
  • Must be a resident of Ontario, and a Canadian citizen or permanent resident of Canada 
  • For new residents in Ontario, there is a 3 month waiting period before eligibility begins
Application
  • Must attend in person for a photo health card 
  • Original documents must be provided such as proof of citizenship, Ontario residency (call for list of documents acccepted, with proof of identity, including signature) 
  • For individuals under 16, a parent or guardian must attend and bring the child's birth certificate or citizenship card, as well as their own documents
