Description (Service) Provides programs and services to meet the needs of individuals of all ages with an intellectual and/or developmental disability
 
Provides the following programs: 
  • Children's Services 
  • Children's Services, Child and Youth Program 
  • Children's Services, Infant Development Program 
  • Community Mental Health Support Services 
  • Community Mental Health Support Services, Wellness Project 
  • Community Mental Health Support Services, Housing Subsidy Program 
  • Kid's Zone Childcare Services 
  • Options for Adults Program, 24 Hour Intensive Support Residences 
  • Options for Adults Program, Enriched Supported Independent Living 
  • Options for Adults Program, Host Family 
  • Options for Adults Program, Community Participation Services and Supports
 
Individual Support Plan (ISP) 
  • Identifies what individuals needs and wants are in relation to life in general (assistance from family requested) 
  • Identifies individual's personal, educational, vocational, medical, and safety support needs in relation to wants 
  • Connects and supports individuals in accessing resources to meet identified needs and wants of the individual
 
Transitional Services  
  • Provides support to youth with developmental disabilities to create a life plan for the transition to life after graduating from high school 
  • Assists with various applications such as housing, passport, special services and Ontario Disability Support Pension 
  • Offers guidance for accessing further education, employment and training opportunities and community resources 
  • Advocates on behalf of the family and individual
Hours Mon-Fri 8 am-4 pm

Transitional Services
 
Individuals graduating from high school; support can begin anytime during school years
