Description & Services
Description (Service) Provides Intake and Service Coordination for children's service agencies and adult developmental services
 
Children's Case Manager Program 
  • Provides information and referrals to other services including physiotherapy, occupational therapy, counselling, psychology, telepsychiatry, child development and community integration 
  • Provides case management to ensure children and youth are provided with services they require in a coordinated, effective and efficient manner
 
Adult Protective Service Worker Program 
  • Provides support to adults with a developmental disability who are living on their own so that they may live as independently, safely and securely as possible in the community 
  • Offers advocacy to develop greater independence and inclusion in the community 
  • Provides case management to ensure that individuals are provided with services they require in a coordinated, effective and efficient manner
 
Planned Out-of-Home Respite 
  • Provides funding for the family to hire a respite worker to care for their high needs child outside of the family home 
  • Enables families to rest, attend to personal needs, or participate in outside activities

Eligibility Children's Case Manager Program
 
Children 0-18 years of age
 
Adult Protective Service Worker Program 
  • Must be at least 18 years of age and have a developmental disability 
  • Must satisfy diagnostic criteria
 
Planned Out-of-Home Respite 
  • Children who have high, multiple, special needs 
  • Children under 18 years of age and living at home with their parents 
  • Must have more than one disability related need resulting from a physical and/or developmental disability 
  • Requires support for participation in activities of daily living, school and play
