Description (Service) Provides Intake and Service Coordination for children's service agencies and adult developmental services
Children's Case Manager Program 
  • Provides information and referrals to services and supports 
  • Provides case management to ensure that children and youth are provided with services they require in a coordinated, effective and efficient manner
Planned Out-of-Home Respite 
  • Provides funding for the family to hire a respite worker to care for their child outside of the family home 
  • Enables families to rest, attend to personal needs, or participate in outside activities

Planned Out-of-Home Respite 
  • Children who have high, multiple, special needs 
  • Children under 18 years of age and living at home with their parents 
  • Must have more than one disability related need resulting from a physical and/or developmental disability 
  • Requires support for participation in activities of daily living, school and play
