Description (Service) Provides services that promote physical and mental health for community members 
  • Provides health and wellness awareness and education, activities and training to meet health needs within community 
  • Facilitates once-weekly services of a visiting Physician 
  • Coordinates clinics through the Community Health Nurse in areas that may include Diabetes, Immunization, Sexual Health and Communicable Diseases 
  • Offers prenatal and postpartum health care  
  • Provides smoking cessation programs and counselling upon request  
  • Provides traditional ceremonies for health issues, information about traditional herbs and medicines and consultations with traditional healers 
  • Works with the Northshore Tribal Council health department for the provision of primary health care, chronic disease nurse, dietitian, mental health counselling and physiotherapy
 
Addresses individual health care needs by providing: 
  • Intake services to determine health care needs of individual  
  • Referrals to appropriate health care providers based on information in the intake assessment 
  • Advocacy on behalf of individual with other health professionals  
  • Education and information about Fetal Alcohol Syndrome (FAS) and Fetal Alcohol Effects (FAE) 
  • Home visits to the sick, elderly, new mothers and persons with health care needs 
  • Mental health counselling, case management and referrals to other service providers and programs 
  • Assistance with coordinating local and long distance travel arrangements for medical appointments

  • Community Health Representative 
  • Community Support Services Worker 
  • Community Support Services Family Preservation Worker 
  • Diabetes Program 
  • Home and Community Care Program 
  • Medical Transportation/Non-Insured Health Benefits 
  • Traditional Coordinator
