Description (Service) Provides a wide range of services for children, youth, and adults with intellectual disabilities and/or autism spectrum disorders and their families

  • Assists individuals to live full and complete lives within their community 
  • Offers Shared Living 
  • Offers Community Homes Program 
  • Offers Supported Independent Living 

    Community Participation Supports 
  • Offers a variety of options according to individual choice 
  • Enhances increasing social roles and volunteer opportunities 
  • Assists with pursuing leisure activities within the community
    Community Supports 
  • Provides case management and advocacy support to adults and children living in the community 

  • Assists with finding meaningful employment 
  • Offers job readiness, resumé writing, job searches skill development and job coaching 
  • Provides Vocational Supports and Employment Services 
  • Offers career planning for young adults 

    Specialized Supports 
  • Supports according to individuals' needs at a given time in their life

    Ontario Telemedicine Network (OTN)  
  • Provides access to specialized medical care using video conferencing and other tele-diagnostic equipment  
  • Allows specialists to remotely examine and prescribe treatments to patients in their home communities  
  • Reduces the need to travel to receive medical care

  • Children's Services-Diagnosis of Intellectual Disability or Autism Spectrum Disorder by a registered medical practitioner 
  • Adult Services-application process through Developmental Services Ontario
