Description (Service) Offers a wide range of community health care solutions including the following services:
 
The Adult Day Centre  
  • Assists individuals in maintaining their independence in the community for as long as possible 
  • Provides relief for caregivers of frail elderly and socially isolated seniors
 
Volunteer Hospice Service 
  • Offers non medical palliative volunteer support to persons living with a terminal illness and their family  
  • Visits in home or hospital or other facility 
  • Support patients emotionally, spiritually and practically 
  • Provides respite to caregivers and bereavement support
 
Bereavement Support Services 
  • Provides a 9-week bereavement support groups for adults 
  • Provides monthly open information sessions offering education on bereavement issues
 
Visiting Nursing Services  
  • Provides in home nursing services for health care issues, including foot care 
  • Offers flu clinics in workplace
 
Palliative Pain and Symptom Management Consultation Services 
  • Provides expertise and consultation for health care professionals in managing symptoms and supporting individuals with a life threatening illness and their caregivers
