Description (Service)
  • Offers health services and a variety of health programs from an Outreach site 
  • Provides access to other health care professionals within and outside of the Family Health Team 
  • Coordinates documentation if patient receives care from a member of the team
 
Provides the following programs and services: 
  • Chronic Disease Self Management Program 
  • Chronic Pain Self Management Program 
  • Diabetes Program 
  • Mental Health Program 
  • Pharmacist Services 
  • Psychiatry Assessments 
  • Sleep Oximetry Program 
  • Smoking Cessation Program
 
Offers health care services including but not limited to: 
  • B12 injections 
  • Dietitian 
  • Exercise groups 
  • Ear flushes 
  • Immunizations 
  • Wound care
Hours Mon-Fri 9 am-12 noon, 1 pm-5 pm

  • Call for appointment 
  • No referral required for most services 
  • Referrals from physician/nurse required for counselling services 
  • Pharmaceutical counselling/medication checks available in a private setting on Wednesdays; appointment required
