Description (Service) Provides education and services for the prevention, management and treatment of diabetes 
  • Provides case management and screening services 
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes 
  • Advises on developing and implementing an approach to care and management of diabetes 
  • Communicates patients' concerns to family physician 
  • Assists individuals in finding appropriate services 
  • Provides referrals to other health care professionals and the Northern Diabetes Health Network
 
Compassionate Medication Program 
  • Provides diabetes medications (insulin, test strips etc) for individuals without medical coverage
Hours Mon-Fri 9 am-12 noon, 1 pm-5 pm
Areas Served Atikokan
Eligibility Individuals diagnosed with diabetes or high-risk
 
Compassionate Medication Program 
Individuals without medical coverage or cannot afford medications
Application Requires referrals from Physician
