Description & Services
Description (Service) Provides mental health services and support 
  • Offers case management services including individual assessment and treatment planning 
  • Provides supportive mental health counselling to community at large 
  • Offers mental health support to all programs of the Family Health Team 
  • Conducts home visits for individuals who have trouble leaving their home  
  • Offers accompaniment to community activities and appointments 
  • Provides referrals to local and out of town community service providers 
  • Provides individual and public education and information sessions
 
Also provides: 
  • Advocacy on various committees and boards for increased mental health programs 
  • Individual bereavement and grief counselling 
  • Coordination of a mindfulness group to teach patients better stress management
Hours Mon-Fri 9 am-12 noon, 1 pm-5 pm

Eligibility Ages: 16 year(s) and up 
 
  • Individuals experiencing mental health issues; do not require a formal diagnosis 
  • Will provide referrals to required agencies for children under 16 years of age
