Description (Service)
  • Provides mental health services to individuals who are experiencing serious mental illness and require a more intensive approach to assessment, treatment, and rehabilitation 
  • Seeks to optimize an individuals independence and integration into the community 
  • Offers psychiatric assessment and treatment for a period of 6 to 12 months 
  • Provides follow-up after an individual has been discharged and is making the transition to community living
 
Note: Follow-up is provided by the Community Outreach Service and offers support to the individual and family members
Hours Office hours: Mon-Fri 8:30 am-4:30 pm 
Mon-Sun 24 hours
Areas Served Nipissing District
Eligibility Ages: 18 year(s) and up 
 
Individuals must: 
  • Have a diagnosis of a serious mental illness 
  • Reside within the catchment area of the North Bay Regional Health Centre 
  • Be medically stable 
  • Have exhausted district level services 
  • Have a signed letter of commitment from the referring agency/service
Application Referral package must be completed by a physician
