Description (Service)
  • Provides outreach, support, assistance, and intensive case management to individuals with serious mental illness and addictions who come into conflict with the law 
  • Operates in close collaboration with key partners such as the provincial Crown Attorney, North Bay Regional Health Centre Forensic services, lawyers, police services and other mental health service providers
 
Services include: 
  • Release from Custody: includes coordination of required services, discharge planning and referral to community services 
  • Mental Health Case Management-Justice: includes ongoing assessments, treatment planning, coordination of required services, one on one, group and family counselling, conflict resolution and crisis prevention, housing supports, discharge planning, referral to community resources, Court Diversion Order coordination 
  • Transitional Case Management-Forensic: includes ongoing assessments, treatment planning, coordination of required services, conflict resolution and crisis prevention, housing supports, discharge planning and referral to community resources 
  • Mental Health Youth Court Worker: includes ongoing assessments, treatment planning, coordination of required services, one on one, group and family counselling, conflict resolution and crisis prevention, housing supports, discharge planning, referral to community resources, and Court Diversion Order coordination

Eligibility Ages: 12 year(s) and up 
 
Individuals with serious mental illness and addictions who come into conflict with the law 
  • Release from Custody: for individuals being released from jail 
  • Mental Health Case Management-Justice: for individuals who are in conflict with the law for minor offences 
  • Transitional Case Management-Forensic: for individuals who have been found not criminally responsible and are on Ontario Review Board Orders 
  • Mental Health Youth Court Worker: for youth 12-16 years of age and in conflict with the law for minor offences
