Service: Aging At Home Program

Description & Services
Description (Service) Provides services and support to elders with health care needs to enable them to stay at home as long as possible
 
Provides the following services as needed: 
  • Assesses and supports the client at home and out of hospital to prevent repeat admissions to the Emergency Department 
  • Facilitates timely discharge of clients from acute care hospitals 
  • Provides referrals to other health services such as short stay, rehabilitation, long term care or hospice for palliative care 
  • Delivers educational sessions to Personal Support Workers at all seven First Nations on Manitoulin Island 
  • Mentors Registered Nursing staff in palliative, pain, wound and capacity assessments, and placement issues as they provide care for elders 
  • Acts as a resource for family members who are caring for an elderly loved one 
  • Coordinates the Aging At Home Transportation services 
  • Offers the use of an electric bed with special mattress as well as positioning aids and mobility aids for palliative or bedridden clients 
  • Rents the use of a negative pressure vac unit which expedites healing of wounds

Areas Served Aundeck Omni Kaning First Nation ; Little Current ; M'Chigeeng First Nation ; Sheguiandah ; Sheshegwaning ; Whitefish River First Nation ; Wikwemikong ; Zhiibaahaasing First Nation
Eligibility Ages: 55 year(s) and up 
 
All Manitoulin First Nations elders and the off-reserve Aboriginal elderly population
