Manitoulin Island Treaty
Description (Service) Provides education and services for the prevention, management and treatment of diabetes  
  • Provides case management and screening services  
  • Provides basic foot care services 
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes  
  • Advises on developing and implementing an approach to care and management of diabetes including monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices 
  • Assists individuals in finding appropriate services  
  • Provides referrals to other health care professionals as needed
Hours Mon-Thu 8:30 am-4:30 pm, Fri 8:30 am-12 noon

Eligibility
  • Members of Zhiibaahaasing First Nation 
  • Adults 18 years and older with pre-diabetes, type 1 diabetes, and type 2 diabetes
