Description (Service) Provides education and services for the prevention, management and treatment of diabetes  
  • Provides case management and screening services  
  • Provides basic foot care services  
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes  
  • Advises on developing and implementing an approach to care and management of diabetes  
  • Assists individuals in finding appropriate services  
  • Provides referrals to other health care professionals as needed
Hours Mon-Fri 8 am-12 noon, 12:30 pm-4 pm

Eligibility Members of Wikwemikong Unceded Indian Reserve living with diabetes or at-risk
