Description (Service)
  • Ensures health services to communities are accessible and appropriate 
  • Works with communities to coordinate, meet the needs and improve the health and well-being of First Nations people
Health care professionals include: 
  • Home Care Program Nurse Manager and Case Manager 
  • Personal Support Workers 
  • Community Health Nurses 
  • Registered Nurses
Coordination of care services include: 
  • Access to mental health nurse, dieticians, traditional healers 
  • Accompanied doctor visits 
  • Blood draws 
  • Care giver relief (respite) 
  • Companion services (friendly visits) 
  • Foot care chiropodist clinics 
  • Health risk assessment 
  • Monitors and/or administers prescribed medications 
  • Monitor vital signs 
  • Occupational and physiotherapy 
  • Palliative care 
  • PICC and IV line maintenance 
  • Referrals to appropriate health team members
Hours Mon-Fri 8:30 am-4 pm
Areas Served Aundeck Omni Kaning First Nation ; Sheguiandah ; Sheshegwaning ; Whitefish River First Nation ; Zhiibaahaasing First Nation
Eligibility Must be a member of one of the participating First Nations
  • Referral is required; visit website for referral form 
  • Registration can be done with Home Care Program Nurse Manager or with Community Health Nurse at local community Health Centre
