Description (Service)
  • Provides health promotion and prevention strategies on health issues related to diabetes 
  • Provides group education sessions and healing circles or individual counseling 
  • Provides client-centered care by screening, referrals and promotion of diabetes self-management 
  • Offers education, screening, nutrition, and prevention/awareness programs 
  • Offers Diabetes Health Promotion Worker visits in each community twice per month
Hours Mon-Fri 8:30 am-4 pm
Areas Served Aundeck Omni Kaning First Nation ; Sheguiandah ; Sheshegwaning ; Whitefish River First Nation ; Zhiibaahaasing First Nation
Eligibility Must be a member of one of the participating First Nations

  • Referral is required; visit website for referral form 
  • Registration can be done with Home Care Program Nurse Manager or with Community Health Nurse at local community Health Centre
