Description (Service) Responsible for processing Non-Insured Health Benefits service claims and coordinating medical transportation for community members  
  • Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual  
  • Assists with making travel arrangements  
  • Maintains client documentation, receipts and related information  
  • Provides transportation to medical appointments within the community and medical appointments out-of-town in Little Current, M'Chigeeng, Espanola, Sudbury and other centres
Hours Mon-Thu 8:30 am-12 noon, 1 pm-4:30 pm * Fri 8:30 am-12 noon, 1 pm-4 pm

Eligibility Members of Sheguiandah First Nation who require assistance with Non-Insured Health Benefits (NIHB) arrangements and forms, and assistance with medical transportation due to age or disability
Application
  • Must make arrangements for out-of-town travel at least 2 weeks prior to date of appointment/travel 
  • Must have Confirmation of Appointment (COA) stamped/signed and return to Health Centre after appointment
 
Must provide the following information when booking: 
  • Full name, address and telephone number 
  • Date of birth 
  • Status registration number 
  • Date, time and address of appointment 
  • Doctor's name and specialty 
  • Written proof of appointment such as appointment card
