Description (Service) Self-Management and Exercise Program for stroke survivors and their care partners (family and friends)
 
  • Focuses on education, goal-setting, and exercise 
  • Helps individuals learn about strategies to manage the daily challenges of living with a stroke 
  • Holds sessions three times a year, in Jan, Apr, and Sep 
  • Provides up to 6 participants in Thunder Bay (host site) and 2-6 participants at 1-2 regional sites via Ontario Telemedicine Network

  • Stroke Survivors and their care partners 
  • Individuals must have completed their stoke rehabilitation program
