Program operates out of the Band Office in Red Lake; also offered in Thunder Bay on a consistent basis to members living in the city
Description (Service) Provides education and services for the prevention, management and treatment of diabetes  
  • Provides case management and screening services  
  • Provides basic foot care services  
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes  
  • Advises on developing and implementing an approach to care and management of diabetes  
  • Assists individuals in finding appropriate services  
  • Provides referrals to other health care professionals
Hours Mon-Fri 9 am-5 pm

Eligibility Members of McDowell Lake First Nation living with or at-risk for diabetes
