Description (Service) Specialized interprofessional teams including Nurse Practitioners, Occupational Therapy, Physiotherapy and Dietitian to provide a coordinated approach to diabetes management and treatment  
  • Provides case management and screening services  
  • Offers basic and advanced foot care services  
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes  
  • Advises on developing and implementing an approach to care and management of diabetes  
  • Assists individuals in finding appropriate services  
  • Provides referrals to other health care professionals

Patients with diabetes and related, collective health issues that require more intensive treatment strategies
 
Services and programs for any of the following: 
  • Persons with Type 1/Type 2 Diabetes 
  • Gestational Diabetes 
  • Type 1/Type 2 Pregnancy 
  • Impaired Glucose Tolerance 
  • Impaired Fasting Glucose 
  • High Risk persons

  • Requires booked appointments 
  • Physician or nurse practitioner referral required
