Description & Services
Description (Service) Offers supports to individuals with intellectual disabilities living in their own homes in the community, either alone or with a roommate 
  • Tailors supports through the creation of an Individual Support Plan (ISP) 
  • Provides case management through individual Case Managers 
  • Works to meet all aspects of the individuals needs, including but not limited to personal hygiene, cooking, cleaning, socialization, financial management and health 
  • Provides one to one personal support within the home or community  
  • Teaches and/or maintains daily living skills, problem solving skills  
  • Determines staff supports based on the needs of the individual receiving supports and their families 
  • Encourages independence and self-determination
Hours Office Mon-Fri 8 am-4 pm 
Services are available days, evenings and weekends as required

Eligibility Individuals with an intellectual disability who do not require 24-hour support
