  • Provides escorted transportation to medical appointments, shopping, and to participate in various social activities and programs 
  • Participants must meet the eligibility requirements

Eligibility Ages: 18 year(s) and up 
 
  • Individuals must have specific needs such as disability, illness or injury that require assistance or are seniors 65 years of age and older 
  • Medical appointments are a priority
Application
  • Requires completion of application or be registered with the program 
  • Must make arrangements at least 48 hours in advance  
  • Cancellation notice should be given at least 24 hours in advance if possible
