Description (Service) Provides case management services to individuals and families with mental health and addictions issues
 
Case Management Services include the following: 
  • Client directed goal planning 
  • Counselling 
  • Crisis intervention and support 
  • Client advocacy and service coordination 
  • Family support and education 
  • Individual assessment 
  • Medication monitoring 
  • Skills development
Hours Mon-Fri 8 am-4 pm
Areas Served Ear Falls ; Red Lake
Eligibility Ages: 18 year(s) and up 
 
Individuals living with serious mental health issues
Application
  • Open to referrals from any source including self-referral 
  • Intake assessment required, by phone or walk-in 
  • Services all confidential
