Description (Service) Provides education and services for the prevention, management and treatment of diabetes 
  • Provides case management and screening services  
  • Offers basic foot care  
  • Offers walking programs and exercise programs tailored to meet the needs of the individual 
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes 
  • Advises on developing and implementing an approach to care and management of diabetes, healthy meal plans and blood glucose monitoring 
  • Assists individuals in finding appropriate services  
  • Provides referrals to other health care professionals
Hours Mon-Fri 8 am-4:30 pm
Areas Served Nipissing First Nation
Eligibility Members of Nipissing First Nation with diabetes or at-risk
