  • Provides support to youth with intellectual/developmental disabilities to create a life plan for the transition to life after graduating from high school 
  • Assists with various applications such as housing, passport, special services and Ontario Disability Support Pension (ODSP) 
  • Offers guidance for accessing further education, employment and training opportunities and community resources 
  • Assists with various living arrangements such as apartment searches, shopping, banking, food preparation, and medical appointments 
  • Advocates on behalf of the family and individual
Hours Mon-Fri 8 am-12 noon, 1 pm-4 pm
Areas Served Cochrane ; Kapuskasing ; Includes communities along Hwy 11 from Opasatika to Cochrane
Eligibility Ages: 18 year(s) - 25 year(s) 
 
Individuals with an intellectual and/or developmental disability making the transition from school to community life; support can begin anytime during school years
