Description (Service) Offers support services for people living with prediabetes or diabetes 
  • Provides individual and group sessions on diabetes related issues 
  • Services are provided by a Registered Nurse and Dietitian
 
Provides the following services: 
  • blood pressure checks 
  • cholesterol checks 
  • foot assessments/care 
  • insulin adjustments 
  • nutritional and weight management education 
  • resource materials
 
Also provides the following diabetes education: 
  • Diabetes and Chronic Disease Self Management Workshops 
  • Foot Care Education Workshop 
  • Insulin Class
Hours Mon-Fri 8 am-4 pm
Areas Served King Kirkland ; Kirkland Lake ; Larder Lake ; Matachewan ; Matachewan First Nation ; Swastika ; Virginiatown
Eligibility Ages: 18 year(s) and up 
 
  • Individuals and families living with or affected by diabetes 
  • Must provide proof of valid provincial health care (OHIP) coverage
