Description (Service) Provides support to individuals struggling with a combination of mental illness and abuse of alcohol or other substance (concurrent disorder)  
  • Provides case management services 
  • Provides a treatment plan based on a comprehensive assessment, focused on identifying treatment needs and issues in the areas of both substance use and mental health 
  • Facilitates psychiatric consultation and follow-up 
  • Provides family support and education 
  • Assists individuals with concurrent disorder issues to navigate the complex system of community, health and social services 
  • Offers aftercare services
Hours Office Mon-Fri 8:30 am-5 pm 
Community Mental Health Sites: Each one open Mon-Fri 8:30 am-5 pm

First Nation members and residents of the Weeneebayko region struggling with co-occurring mental illness and alcohol/substance abuse
