Description (Service) Supports for people living with pre-diabetes or diabetes 
 
  • Supports people living with/affected by diabetes by providing education that assists with the treatment and management of diabetes 
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes  
  • Partners with other programs and departments to provide the best possible diabetes care
 
Offers the following services: 
  • Assistance with accessing specialty services such as chiropody, ophthalmology & pediatric diabetes treatment 
  • Diabetes information and education 
  • Diet and nutrition education 
  • Managing diabetes with on-going support and assistance 
  • Understanding diabetes treatment options
Hours Office Mon-Fri 8 am-4:30 pm
Areas Served Attawapiskat First Nation ; Fort Albany First Nation ; Kashechewan First Nation ; Moose Factory ; Moosonee ; Weenusk First Nation - Peawanuck
Eligibility First Nation members of the Weeneebayko region living with diabetes or at-risk
Application Services provided at a local level in each community
Languages English ; Interpretive Services - Cree, available upon request ; Handouts are also available in Cree
